STATUS AND PROSPECTS OF RESEARCH IN PEDIATRIC SURGERY

Abstract

The paper presents a systematic analysis of the status and prospects of development of scientific research in pediatric surgery. Separately the modern opportunities of research funding within the framework of state programs and grants, research potential and the organisation of children’s surgical research in the country on a sample of 20 University Clinics. Authors showed the implementation of the principle of the scientific-educational cluster, the performance of dissertational works, scientific forums of pediatric surgeons. Over the past five years by pediatric surgeons were protected 97 theses, including 22 for the degree of doctor of science; every fifth thesis was performed at the junction of specialties. The analysis of 200 scientific publications in Central and regional magazines and 69 of the patent, defined the vector of priority of publication in journals with high impact factor registered in Scopus and Web of Science. A separate analysis was subjected to international cooperation of pediatric surgeons - 91 report abroad for five years, participating in congresses EUPSA and WOFAPS. Conclusions are drawn about the feasibility to build scientific work in the framework of the Russian Association of pediatric surgeons, combining the efforts of all staff, their potential and capabilities by intelligently organizing research, and presenting children’s surgery in the relevant authorities. Authors have formulated tasks for the development of pediatric surgical science perspective.

USE OF FRESH AUTOLOGOUS CELLS IN CHILDREN'S COMBUSTIOLOGY PRACTICE

Abstract

The purpose of this research is to assess the efficiency of the original technology based on combining fresh autologous skin cells and fibrin glue in treating 2nd and 3d degree burns. This technology was used in 104 children with the burn area from 10 to 80% of the body surface, who received treatment in the burn centre of Privolzhsky Federal Medical Research Center of the Ministry of Health of Russia. The status of patients and their wounds was assessed using clinical observation, visual characterizing of the wound surface, photo registration, planimetry, IR imaging, and laser doppler flowmetry. The cell material was characterized using microscopic methods, specifying cell viability and concentration. Cell phenotype was determined by the cytofluorometry method. For 2nd degree burns, cell suspension and fibrin glue were applied directly to the wound after a primary toilet; for 3d degree burns - to the wound bed formed after an early necrectomy. The use of this technology made it possible to reduce the number of dressings and improve the quality of a newly formed skin integument in treating 2nd and 3d degree burns. The developed method is effective, safe and requires no additional costs; it can be used in any surgical hospital.

RENAL VENOUS HYPERTENSION AS A RESULT OF PORTOSYSTEMIC SHUNTING IN CHILDREN WITH PREHEPATIC PORTAL HYPERTENSION

Abstract

Purpose of the study - to study influence of portosystemic shunting on renal hemodynamics in children with portal hypertension. A retrospective analysis of 152 portosystemic shunting operations performed. Impact of portosystemic shunting on renal hemodynamics analyzed by ultrasound Doppler of renocaval segment, multislice computed tomography and magnetic resonance imaging angiography of abdominal aorta. The most frequently we performed proximal splenorenal anastomosis with splenectomy (61.1%). Signs of renal venous hypertension in patients with prehepatic portal hypertension detected in 13 (8.5%) patients. In 9 (5.9%) cases, the cause was congenital anomalies of renocaval segment, most of which (n=7) circumaortic left renal vein. Inadequate reduction of esophageal varices after portosystemic shunting was observed in 6 (46%) of 13 patients with symptoms of renal venous hypertension. However, only 1 patient with circumaortic left renal vein due to thrombosis of distal splenorenal shunt reoperated - mesocaval bypass. Portosystemic shunt operation using the anomalous left renal vein can cause unsatisfactory results with the development of rebleeding from esophageal varices. Besides a large amount of blood drained from the portal system in to left renal vein can induce nutcracker phenomenon, and as a consequence the development renal venous hypertension.

EARLY ENDOSCOPIC CORRECTION OF VESICOURETERAL REFLUX IN CHILDREN WITH SPINA BIFIDA

Abstract

The article discussed preliminary results of endoscopic correction of vesicoureteral reflux in children using polyacrylate polyalcohol copolymer (Vantris). Injections of polyacrylate polyalcohol copolymer were distributed to 33 patients of the urology department of the Filatov Pediatric Hospital between 2009 and 2014. There were 18 boys and 15 girls of average age of 3.5 years (between 3 months and 16 years). Fourteen patients had bilateral reflux (42.4%) and nineteen patients had unilateral reflux (57.6%). There were 47 injected ureters. Eighteen patients had IV grade reflux (38.9%), 22 patients had III grade reflux (46.2%), and seven patients had II grade reflux (14.9%). Injected volume per unit was 0,76 ± 0,43 ml. Follow up median was 20 months. Reflux was eliminated in 38 renal units (76.7%), decreased to grade I-II in 9 (19.1%) and persisted in 1 (2.2%). One patient experienced uretral obstruction (2.2%). It was treated with Cohen transtrigonal ureteric reimplantation surgery. Overall success level was 95.6%.
